What to Prepare for Your Appointment
Valid, Unexpired Government-Issued Photo ID
Florida law strictly requires every signer to present an acceptable form of identification. It must be unexpired, contain a photo, a serial/document number, and a signature.
Acceptable IDs include:
Driver's license or ID card issued by Florida or another US state
US Passport or Passport Card
Foreign Passport (stamped by US Citizenship and Immigration Services)
US Military ID
Permanent Resident Card ("Green Card")
The Complete Document
All Pages Included: You cannot notarize a signature page by itself. You must see the full document from page 1 to the end.
No Blank Spaces: All fill-in-the-blank sections (dates, names, addresses) must be completed before you notarize.
Unsigned: The signature line must be left blank so they can sign it directly in front of you. If the document requires an oath/jurat, wait to sign until the notary arrives. For acknowledgments, if already signed, you must confirm your signature in person.
Physical Presence & Mental Awareness
Physical Presence: The signer must be physically in the room with you.
Awareness & Willingness: The signer must be alert, coherent, and signing of their own free will. This is especially important for senior living, hospice, and hospital visits.
Witnesses (If Required by the Document)
In Need Of Witnesses? If you do not have any witnesses we can provide them to you.
Who Needs Witnesses? Florida Real Estate Deeds, Wills, Trusts, Healthcare Surrogates, and Power of Attorney (POA) documents typically require 1 or 2 independent witnesses.
Witness Requirements: Witnesses must also be physically present, mentally competent, and hold a valid government-issued photo ID.
